LONG BEACH, CA, UNITED STATES, October 13, 2026 /EINPresswire.com/ -- SCAN Health Plan (SCAN), one of the nation’s largest not-for-profit Medicare Advantage Plans, and CareMore Health, (“CareMore”), a Mosaic Health company with over 30+ years leading provider integrated medical care, today announced the launch of CareMore Health Plan by SCAN (HMO), a new provider-specific Medicare Advantage plan designed to bring coordinated care, personalized support, wellness programs and expanded access to older adults in Southern California.

“SCAN and CareMore share a deeply held belief that healthcare should be built around people, not institutions,” said Dr. Sachin H. Jain, president and CEO of SCAN Group and SCAN Health Plan. “CareMore helped shape my own understanding of what is possible when care is proactive, personal and grounded in lasting relationships with patients. Bringing our organizations together gives us an opportunity to pair that approach with SCAN’s expertise in serving older adults and create a Medicare Advantage experience where coverage and care work more closely together."

CareMore has served Southern California communities, delivering relationship-based care centered on prevention, chronic disease management and strong connections between patients and their care team. Through this approach, patients receive longer visits designed to enable more comprehensive diagnosis and management of complex health conditions, tailored personalized care plans and increased focus on preventive screenings to help patients stay healthier and maintain their independence.

“For more than 30 years, CareMore has believed that great healthcare starts with truly knowing the people we serve,” said Sam Wald, Chief Executive Officer of CareMore Health. “By joining efforts with SCAN, we can bring together CareMore’s personal approach to care with SCAN’s deep experience serving Medicare-eligible patients. Together, we’re making it easier for people across Southern California to get the care, support and personal attention they need to live healthier lives.”

The collaboration brings together two organizations with decades of experience serving Medicare patients and a shared commitment to helping them live healthier, more independent lives. Founded by seniors in 1977, SCAN has spent nearly 50 years focused on the needs of older adults, while CareMore has more than three decades of experience delivering proactive care for older adults and people with complex health needs.

This also marks a full-circle moment for the organizations' leaders. SCAN Group and SCAN Health Plan CEO Dr. Sachin H. Jain served as president and CEO of CareMore from 2015 to 2020, while CareMore CEO Sam Wald previously held a leadership role at SCAN. Their shared history reinforces a broader alignment between SCAN and CareMore around knowing the people they serve, anticipating their needs and helping them stay healthy and independent.

CareMore Health Plan by SCAN will be available to eligible Medicare beneficiaries in California’s Los Angeles and Orange counties for coverage beginning January 1, 2027. Medicare’s Annual Enrollment Period runs from October 15 through December 7, during which Medicare-eligible older adults can enroll in the plan for 2027 coverage.

* Prescription drug costs may vary depending on the plan, county, pharmacy type, days’ supply, Part D benefit phase, and whether the member receives “Extra Help.”

SCAN Health Plan is an HMO plan with a Medicare contract. Enrollment in SCAN Health Plan depends on contract renewal.

Other providers are available in SCAN Health Plan’s network.
